I can no longer eat any tomatoes and any foods or sauces with a tomato base. Has anyone else experienced this with Mestinon, and is there a simple remedy?

I have yet to hear anyone mentioning they experienced this while on mestinon. However, mestinon itself commonly leads to these side effects (https://myasthenia-gravis.com/living/mestinon-side-effects-relief, https://myasthenia-gravis.com/forums/have-you-had-adverse-gastrointestinal-issues-from-mestinon-pyridostigmine, https://myasthenia-gravis.com/forums/side-effects-from-pyridostigmine-mestinon) --- How many times have you attempted to eat tomatoes or tomato-based food since you started experiencing stomach cramps and diarrhea? --- Those are some symptoms of a tomato allergy, which can develop later in life due to the immune system or possibly be triggered by other factors. In your case, the acidity might suddenly have started to irritate your stomach lining and cause the symptoms you're experiencing. Tomatoes are known to be a possible trigger for IBS.
