Ocular MG transitioning into Generalized?

I currently have Ocular Mg. Not bad. I get double vision and prednisone makes it go away. It now takes 4 days of 60mg then weaning. The flares were coming every 6 1/2 weeks. I was taking Mestinon of course and now also getting Rystiggo infusions. I am starting my 3rd 6 week dose. Since the Rystiggo I have had 2 flares 8 1/2 and 13 weeks apart. Improvement! However after my last eye flare, my eyes did not totally clear up. They are off just a little. I can see clearly but only at certain angles. Doesn't bother me much at all. However I have another issue now. Low energy! Especially noticeable if doing anything strenuous. For instance, I was able to fish very hard for 4 hrs in my kayak today, but when I went to load it in my car I got extremely tired and had to sit down on the ground for a few minutes afterward. I have noticed similar issues doing yard work this past week. I get tired after only 30 minutes and extremely "sit down" tired if doing anything strenuous. Luckily I have a regular scheduled appointment with my neurologist in 2 wks. Does this sound like my MG is transitioning to Generalized? If you had Ocular MG and it transitioned to Generalized, how did it occur?

  1. Yes, it sounds like genialized weakness in your fatigable muscles. Most ocular MG usually evolves into GMG. They are both the same disease, just distinguished by what muscles they attack. Talk to your neurologist. You may need some additional medication. And be careful exerting yourself, especially if you are out alone. The nature of GMG is that muscles get weaker with exertion. Mine sometimes shut down completely, and I am paralyzed for a few to several minutes. I still live a great life, just within some reasonable limits. I'm 76.
